Welcome to the Frostburg State University family! We look forward to providing an atmosphere in which you will continue to excel academically. In anticipation of your arrival on campus, we have provided the following checklist to assure that your transition to the University is a smooth one. To complete the following forms you will need your

  • Student ID
  • Username
  • Password

Your Student ID was included in your acceptance letter.

Information on your FSU username and password

These forms must be completed through the Patient Portal by August 1st (Fall Semester) and January 2nd (Spring semester).

*The required records MUST BE scanned and uploaded through the Patient Portal. Our system is all electronic. NO mailed, fax or email copies of immunization records are accepted.*

Complete and upload the following through the Patient Portal:

University Immunization Compliance Policy:

Immunization Compliance Requirements- Immunization Record and tuberculosis Assessment (TB screen) documents must be legible and uploaded through the Patient Portal
Immunization Record must be uploaded through Patient Portal. Follow instructions at the top of the Immunizations & Vaccines page.
TB Screen (Tuberculoses Risk Assessment). (required to fully utilize Patient Portal. Complete the Tuberculosis Risk Assessment Form as well as other forms on the Patient Portal, once you have received your FSU user name and password. Additional information required for any positive response.
Consent to Treat Minor Form (parent signature needed if student will be under 18 at date of first attendance).
 
Forms required to be filled out in order to fully utilize health center services (required for all appointments): Consent for Treatment, Telehealth Consent, Notice of Privacy, Medical History / Social History

ALLERGY INJECTIONS

If you need to receive allergy injections, complete and submit these forms to the Student Health Center for review.

Note: If you are a new student, you will not have access to this portal until at least 48 hours after your acceptance fee has been paid and processed.

If you are having difficulty locating your immunization and are a resident of Maryland, you can register with Myirmobile to see if your provider has your immunization on record with myirmobile.
